These refrigerator magnets and wallet size pouches that contain your medical information are available FREE for all Clay County seniors. Extra inserts are available too.

Instructions for filling out File of Life cards

1. Fill out the insert card with your medical, health, and emergency contact information.
2. Place the larger one on the outside of your refrigerator.
3. Carry the smaller one in your purse, wallet, or glove compartment.
4. Be sure to update this information each time your medications or health condition changes.
